Documentation Requirements
The following are descriptions of the types of documentation required to receive accommodations and supports through The Center for Accessibility and Neurodiversity (CAN).
Documentation submitted to CAN will be used for the expressed purpose of determining eligibility for services, determining appropriate accommodations, and ensuring effective implementation of those accommodations. All information submitted will be protected against misuse by others.
All documentation that is not provided on one of the linked forms suggested in the areas below should be typed on the provider’s letterhead, dated and signed by the provider.
Please note that an IEP or 504 Plan may be used to meet some documentation requirements. Please see notes under each specific disability category to determine if providing an IEP or 504 Plan is an option.
Learning Disability
A full psycho-educational assessment is helpful, but other documentation that includes the following may be considered:
- Diagnosis
- Date of initial diagnosis and date of the last contact with student
- An assessment battery (with scores), including:
- Aptitude/cognitive testing
- Individual Academic Achievement Testing
- Information Processing Testing
Clinical Summary Addressing:
- How has the evaluator ruled out other alternative explanations for academic challenges
- How assessment score patterns reflect the presence of a learning disability
- Academic limitations caused by the diagnosis
- The degree to which learning is impacted
- Rationale for any specific accommodations being requested.
An IEP with a psycho-educational evaluation may be accepted as documentation if the information included addresses the above requirements
504 Plans will be reviewed to determine if further evidence or information is required.
ADHD
Please have a physician fill out the Medical Health Verification Form (PDF).

Deaf/Hard of Hearing
Please provide recent audiological documentation from a certified audiologist that is on the provider’s letterhead, date, and signed by the provider.
Documentation must include:
- Diagnosis
- Prognosis
- Academic Limitations (if any)
- Severity of Hearing Loss and Impact of Diagnosis
An IEP may be provided, but a current audiology report will be needed as well.
Visual, Physical, Medical Disabilities
Please have a physician fill out the Medical Health Verification Form (PDF).
Mental Health Conditions
Please have a licensed provider fill out the Mental Health Verification Form (PDF).
Autism Spectrum
Please have a physician fill out the Medical Health Verification Form (PDF).
An IEP with a psycho-educational evaluation may be accepted as documentation if the information included addresses the above requirements.
504 Plans will be reviewed to determine if further evidence or information is required.
