2.1 Therapeutic Massage Application Packet
2.1A Application packets will be available in the Counseling Center or the Admissions and Records Office year-round.
2.1B Completed application packets should be turned into the Admissions Office.
2.1C Application packets are date-stamped as they are received in the Admissions Office.
2.2 Supporting Documents
2.2A All supporting documents (see 2.2B) are required in the Admissions office within one month of submitting the application.
2.2B Necessary Documents
2.3 Transfer Students from Other Colleges/Schools with Therapeutic Massage Coursework
- Official Final Copy of your High School Transcript (also required of those who did not graduate)*
- Official Copy of your College Transcript(s) -DO NOT send an IVCC transcript*
- GED Score Report (if applicable)*
- Two (2) Therapeutic Massage receipts*
- Proof of English and Reading assessment*
*No faxes or photocopies will be accepted
2.3A Application packet to the Therapeutic Massage Program must be submitted directly to the Therapeutic Massage Coordinator, contact Cherie Monterastelli at (815) 224-0507.
2.3B Official transcripts from all previously attended colleges/schools must be sent to the Admissions Office.
2.3C Therapeutic Massage credits will be evaluated on a course-by-course basis. Please submit course descriptions from previous Therapeutic Massage program (especially those on an hours based program, please provide a breakdown of the hours spent for each course).
2.3D All other credits are subject to Illinois Valley Community College transfer policy.
2.4 Licensed Massage Therapists (Advanced Placement criteria for AAS Degree)
2.4A Submit a letter of application for admission to the Therapeutic Massage Coordinator, Cherie Monterastelli (815) 224-0507.
2.4B Students must be licensed to practice Massage Therapy in the State of Illinois; or provide documentation to verify that the application has been completed and submitted to the Illinois Department of Financial and Professional Regulations.
2.4C Licensed Massage Therapists will submit a copy of current license, transcripts from school (if applicable), a letter of recommendation from their immediate supervisor (if applicable), and proof of professional liability insurance.
