2014 Support Staff Distinguished Service Award Nomination Instructions
Please read the information on
this page to nominate a support staff employee for Illinois Valley Community
College’s Support Staff Distinguished Service Award:
may work together on a nomination and submit one form. All names of those contributing to a group
nomination should be listed. An IVCC
employee may nominate only one support staff person for the award.

examples of why the support staff employee deserves this award.

The nominee should be the kind of person
who has outstanding work performance, positive interpersonal characteristics,
and who demonstrates loyalty, respect and commitment to IVCC. All support staff employees who were employed
by IVCC during the preceding calendar year will be eligible to receive the
award. Once an individual has won the
award, he/she will not be eligible for nomination for three years.
Please use the web site or attached document
to provide specific examples that are distinctive to the nominee and that are
in direct correlation to the nominee’s work performance, interpersonal
characteristics, and his/her overall commitment to IVCC. Remember, nominees will not be chosen
according to a nominator’s writing abilities, but will be judged on his or her
achievements and contributions to Illinois Valley Community College based on the
specific examples provided. Petitions
are not accepted, only written comments.
Completed nomination materials are due by Monday, February 17, 2014 to
the IVCC Human Resources Office (C-319).
Please fold your nomination material in half and staple or tape it
