Enrollment at Our Center

Starting the Process

• Submit a completed "Pre-registration Form" and a $20 registration fee.  
• To obtain a "Pre-registration Form," call the center director at 224-0218 or 224-0302.  If you wish, you may also request a complete "Enrollment Packet" at that time.

Prerequisites for Enrollment

• Children must be three years old to attend.
• All forms in the "Enrollment Packet" must be completed and submitted to the center.
• Recent physical (within the last 6 months).
• Up-to-date immunizations.
• Mantoux TB test (administered within last 6 months).
• Evidence of Lead Screening.
• Review of the center's "Parent Handbook."  This handbook contains many important policies and procedures.
